4. Procedures and Methods for the Destruction of Personal Information

The Clinic destroys personal information immediately after the Purpose of Collection and Use has been achieved. The destruction procedure and method are as follows.

5. Rights of Users and Legal Representatives and How to Exercise Them

The membership registration for children under the age of 14 (hereinafter referred to as "Children") is conducted through a separate form written in plain language that is easy for children to understand. Furthermore, we mandatory obtain the consent of a legal representative whenever collecting personal information from Children.

  • Request for access, correction, and deletion of a child’s personal information is available.
  • Direct access, correction, and deletion through the 'Edit Member Information' menu.
  • You may contact the Personal Information Protection Officer via written notice, telephone, or fax.

※ Personal information that is mandatory to be retained by law cannot be corrected or deleted within the statutory retention period, even upon request.

6. How to Withdraw Consent / Cancel Membership

You may withdraw your consent regarding the collection, use, and provision of personal information provided at the time of membership registration at any time.

10. Notice Obligation Following Policy Changes
